How to file for Claim of the deceased
(in Penang)
How long do you have = claim within 1 year
Amount of claim = RM1000.00
1. Get a Form ‘Jenazah’ from any DAP office (office hour 9.30 – 6pm, Mon to Fri)
Prepare :
- Death cert
- IC of claimant
- Birth cert of Claimant
- Bank Statement from Bank of Claimant
2 2. Go to Bank to get them sign on the form
3 3. Bring signed form + above document copies and
submit to DAP office.

Hi Joez
I saw your site on how to claim for deceased in Penang . My mother in law passed away this morning & pleased if you can help me with the claim . My MIL was a Penang resident until a years ago she came down to PJ to stay in Home here & to receive medical treatment. She was also registered for the warga emas under the penang state goverment & had received assistance of RM 100 since 2009 by the way she was 92 when she passed away .
Anywhere i can get the jenazah form on line since i stay in PJ ( X penang )

I have tried to look for the borang permohonan jenazah, can't seem to find. I think you can either email below's contact or go to any nearest ADUN in PJ and ask them to help you.
